How to Complete the End of Training Form and Survey

1. Go to  TPESS.org

2. Hover over the  'Resources' link at the top of the page, then choose the 'Training Completion Form' link. 

3. Type in your training information, your name and then complete the survey based on your training class and add comments.

4. Finally, click on the green 'Submit' button so the notification will go to your trainer to approve your certificate.

5. Once you are approved by your trainer, you will get a confirmation email with your T-PESS certificate attached.  See example email below.
